Antonio Conte's team were two goals to the good after 37 minutes thanks to strikes from David Luiz and Eden Hazard, but Roma pulled off a 33-minute comeback after efforts from Aleksandar Kolarov and Edin Dzeko.
However, with 15 minutes left, Hazard headed in his second of the night to maintain Chelsea's unbeaten record in Group C, while the Blues remain two points ahead at the top of the group.
